👅 Lichen on the tongue
Hello dear patients. Today we are going to review the clinical case of lichen on the tongue. A woman came to us with rashes in the area of tongue, it’s been there for quite a long time. She had been to many clinics, to many medical centers and antibiotics were prescribed to her without any success. In one of places – in a dentist clinic, a correct diagnosis was presumed – this is lichen ruber planus. We see plaques of whitish color on the tongue – presumably, in the beginning they were small nodules and then gradually they turned into small plaques that we see. Sometimes, on the background of these plaques, white color of keratinization develops. Usually they are localized on the dorsum of the tongue as in our case. In literature sources these plaques are sometimes compared with biscuits, powdered by sugar or they are compared with traces on a deep snow cover. These are white spots that we see and between them there are areas of almost normal tissue of the tongue. Here, the diagnosis is called lichen ruber planus. Antibiotics are used sometimes but more commonly, this is a classic treatment with the use of external glucocorticosteroids. They can be used on a tongue with the help of spatula that is wrapped by a bandage. Glucocorticosteroid creams can be used twice a day. Also, there are oral forms – different pastes, however, they are not quite common in Russia. Also, if there’s a high resistance, then a treatment by liquid nitrogen can be tried, however, in mouth it’s related to quite a quick burn, so we try not to use it. For this particular patient, a general treatment was prescribed, intravenous injections of sodium thiosulfate, calcium gluconate were prescribed. Prednisolone injections were prescribed - 10 injections for the first course of treatment. Then we took a break. The effect was not large and then we added retinoids in tablet form as an addition to these injections. This is neotigazon medication. In dosages that are indicated within this treatment, healing came quicker and we saw almost a complete disappearance of such rashes. The combination of classic medications – sodium thiosulfate, calcium gluconate and prednisolone with retinoids is the most efficient today if internal treatment is used and when external methods like hormones and liquid nitrogen do not help. It’s no doubt that in this case, it has to be said that we excluded lesions of tongue by candidae, however, both in PCR analyses and in culture tests for candidae, it was all negative. So we came to a conclusion that this is lichen ruber planus only without addition of mouth cavity candidosis to it.
