The underlying pathology involves altered iron metabolism, reduced erythropoiesis, and shortened RBC lifespan, driven by chronic inflammation. Inflammatory cytokines like interleukin-6 (IL-6) stimulate the production of hepcidin, a liver-derived hormone that blocks iron release from macrophages and suppresses intestinal iron absorption. This leads to functional iron deficiency, where adequate iron stores are present but unavailable for hemoglobin synthesis. Additionally, erythropoietin production is often inadequate, particularly in renal disease, further reducing RBC production. Clinically, ACD manifests with fatigue, weakness, pallor, and symptoms of the underlying disease. Laboratory findings include low serum iron, normal or increased ferritin, and low transferrin saturation, distinguishing it from iron-deficiency anemia.
