Akcja rozgrywa się równolegle do wydarzeń z Resident Evil 2 we wrześniu 1998 roku. Gracze wcielają się w byłą policjantkę Jill Valentine, która próbuje uciec z Raccoon City podczas apokalipsy zombie spowodowanej epidemią wirusa T. Ściga ją inteligentna broń biologiczna znana jako Nemesis-T Type, która próbuje zabić ją i wszystkich pozostałych członków jej oddziału policji, Specjalnej Służby Taktyki i Ratownictwa (S.T.A.R.S.). W grze podążamy także za najemnikiem Carlosem Oliveirą, którego firma Umbrella zwerbowała do pomocy ocalałym The story is set concurrently during the events of Resident Evil 2 in September 1998. Players control former police officer Jill Valentine as she attempts to escape Raccoon City during a zombie apocalypse caused by an outbreak of the T-Virus. She is hunted by an intelligent bioweapon known as Nemesis-T Type, who attempts to kill her and all remaining members of her police division, the Special Tactics and Rescue Service (S.T.A.R.S.). The game also follows the mercenary Carlos Oliveira, enlisted by Umbrella to help survivors.
