In this video we cover, CHF stands for congestive heart failure, a clinical syndrome that occurs when the heart is unable to pump enough blood to meet the body's needs. It can result from a number of cardiovascular diseases, including hypertension, coronary artery disease, and myocarditis.  Drugs used to treat congestive heart failure (CHF) include: Angiotensin-converting enzyme (ACE) inhibitors: These drugs are widely used and act on the renin-angiotensin-aldosterone system (RAAS). Examples include captopril, enalapril, and cilazapril. Beta-blockers: These drugs are less commonly used in children than in adults. Diuretics: These are among the most widely used drugs for CHF. Cardiac glycosides: These drugs are naturally obtained compounds that increase the force of myocardial contraction. Examples include digoxin, digitoxin, oleandrin, bufalin, ouabain, marinobufagenin, telocinobufagin, and aerobufagenin. Mineralocorticoid receptor antagonists (MRAs): These drugs can be used to treat CHF due to their vascular protective effects. Sodium-glucose co-transporter 2 inhibitors (SGLT2i): These drugs are considered one of the "four pillars" of heart failure therapy. Cardiac glycosides are a class of drugs that treat heart conditions like heart failure and arrhythmias: How they work Cardiac glycosides increase the heart's output force and decrease its contraction rate by inhibiting the sodium-potassium ATPase pump in cells. This prevents the cell from pumping sodium out, which increases the amount of sodium inside the cell. This causes calcium to build up in the cell, which increases the velocity and extent of sarcomere shortening, and increases contractility. Examples Some examples of cardiac glycosides include digoxin, digitoxin, oleandrin, bufalin, and ouabain. How they are taken Cardiac glycosides can be taken as a tablet, liquid, or capsule, usually once a day at the same time. Bosentan and tezosentan are both drugs that work by blocking endothelin receptors, but they have different mechanisms of action and uses: Bosentan A competitive antagonist of endothelin-1 receptors, bosentan blocks the binding of endothelin-1 to endothelin-A (ET-A) and endothelin-B (ET-B) receptors. This prevents endothelin-1 from causing blood vessels to narrow, which can improve blood flow and treat pulmonary arterial hypertension (PAH). Tezosentan A vasodilator drug, tezosentan inhibits endothelin receptors, which are overexpressed in many types of cancer cells. Tezosentan can help to dilate blood vessels, improve blood flow, and reduce the workload on the heart. Tezosentan has also been found to have anticancer properties. Nesiritide (Natrecor) is a medication used to treat acute decompensated heart failure in hospitalized patients.
